The CFR says that a simulcasting FM and AM owned by the same licensee and
having the same COL may ID the stations jointly, however it used to state
specifically that the FM station needed to be identified as an FM station,
even if the call letters of the station did not identify it as such.
Looking at the examples you gave, I'm thinking that the problem was probably
related to this. I see nothing wrong with "KABL-AM and FM, Oakland, San
Francisco" (assuming they both had a COL of Oakland) -- you can add any
additional communities to the ID that you want -- but "KABL, Oakland, San
Francisco" does not appear to be a legal identification for the FM station
following the above rule.
