My statement was that John H. was the first =I was aware of= to use a middle initial.  I've 
never heard of these other guys in LA and Denver.

I was aware of Melvin X. Melvin (sometimes known as Melvin X. Ugly), and I should have 
thought of him.  But I don't think he counts, since it's so obviously a made-up name.
