I remember an infamous night at Channel 9 some 40 years ago. The news anchor cooked up the perfect cover to cheat on his wife as he taped the 11 PM news. Only problem was video refused to track properly and thus audio was heard over a slide of the newsman. The station GM went looking for the newsman with a gun.
